That said the HMS Conqueror (gotta love Brit ship names) sank the General Belgrado (ex USS-Phoenix CL-46) in the Falklands War after WWII.
Whenever we participated in war games, they limited our subs area and depth so those ‘sub hunters’ had a chance to find us. If you are on the surface, you are easy to find and target. Once a sub fires a torpedo it may briefly expose your location, but immediate run silent, run deep and you won’t find us.

At least our torpedoes work better than they did during World War 2 (e.g., USS Grunion).
Got that right; our torpedoes included many duds (running under targets, exploding prematurely, etc.) until fixes to the Mark 14 torpedo sometime in 1944 plus introduction of the Mark 18.
Post war studies showed that due to funding constraints, critical testing that BUWEPS wanted to do was curtailed or cancelled all together. The fuzing was a good idea but was pushing the state of the art so there was technical risk that needed to be accounted for.

Facts are important.
It is the first warship sunk by a U.S. torpedo since WW2. Counting non-U.S., it is the third warship sunk by torpedo since WW2. The first was an Indian frigate sunk by a Pakistani submarine in 1971 and the second was the General Belgrano in 1982.

Thus, the mechanism is the detailed planning and assessments done by a joint group including the State Department, Department of War, INTEL and other appropriate elements of the US Government. The State Department is then charged to prepare the “Rules of Engagement” for review and ultimately integration into the Military OPLAN issued to Military Forces down to the lowest levels of command and US Government Agencies involved.

Listening to Caine it sounds like the Iranian Navy is out of business, but he didn’t really mention the IRGCN assets, which tend to be more asymmetric.
The 2 steel hull warships that went down carried surface-to-surface cruise missiles; highly lethal if they aren’t shot down. The Iranian speed boats aren’t as dangerous unless they have a small drone vessel attack capability.
I’m confident we kept location information on the torpedoed vessel. The Navy would not want it getting anywhere near 1. Our carrier in the Arabian Sea (Indian Ocean) 2. Other Navy vessels whether warships or logistics types that resupply them at sea.
