In a series of statements from his Truth Social account, President Trump is recommending that congress bypass the subsides for insurance companies and instead send the funds directly to the healthcare consumers who can negotiate with their healthcare providers.


Medical insurance should be sold nationally, across state lines. The state by state plans are inefficient. The competition and the larger poll of insureds would lower rates.
As far as the ACA COVID subsidies, President Trump should agree to a one-year extension provided federal funds for the subsidies only go to states with voter ID, same day voting laws in place. Otherwise the states can pay for it.
Big insurance won’t go for this.

First, we’ve always needed to separate health insurance coverage from being the responsibility of employers. It’s acts as a “golden handcuff” benefit which is really just a reduction in potential pay re-diverted to health insurance companies.
Second, increase competition so individuals can shop new plans against all others more similar to auto or home. These are not perfect industries but are better than the current health insurance model, and far better then the ACA model which requires purchase from a few major insurers who have regulatory capture full of unnecessary coverages.
Third, reform Medicare and Medicaid. Difficult as so many have paid into an entitlement which the government did not protect and invest but instead spend immediately to cover current costs. Those who have paid in should receive fair additional credits towards the coverages they were promised pre-reform, new younger individuals should be released from the old Medicare and Medicaid payment structure alongside reform, but with greater access to…
Four, a new system where taxes from a reformed system go to need-based vouchers, and only to those who meet reasonable annual requirements including physical and mental health to work below a reasonable set retirement age, drug testing and various fraud checks. No more healthy single lazy dudes or illegal immigrants given precious federal tax dollars. States and cities can vote for and pay for any additional “help”. Federal help should be a minimum in as rare a case as possible once personal wealth, family help, church and community help, and city and state help is used up. Federal vouchers for the truly needy to buy basic plans from competing insurance companies that meet basic, common sense coverage and customer satisfaction regulation.
How good can a “healthcare” system be when none of us can actually avoid our own inevitable death, even with the finest (outrageously expensive) medical care?

The medical payment insurance industry is the BIGGEST reason why it is so expensive. I tried to explain how this works to a Democrat voter who doesn’t believe that an insurer wants the cost of healthcare to be high. If healthcare costs were reasonable, then we wouldn’t need insurance so much.
The same type of person out there refuses to understand the reason why China will pay the tariffs is to maintain market dominance and to prevent domestic production from happening. In my particular discussion, we were talking about PCB manufacture and assembly. If you send your design documents to China for manufacture, aren’t you sending them your technical and potentially secret and proprietary designs to China? Why wouldn’t China want this to continue.
I also commented that China sends to Chinese companies operating in the US which is why no “international shipping” labels are present on PCBs I order from China. They send the shipments to their Chinese company operating in the US and then break out the boxes into their individual orders and submit them for delivery.

I have a family of 4 with a family plan in place since pre “Affordable” Care Act crammed down by Obama. 13 years ago, we used to pay 7k annually with a 3.5k deductible. In 2025, we pay 36k plus a 3.5k deductible. This doesn’t cover dental or vision. We typically spend 45-50k annually on our medical expenses. We’re lucky to have the household income to cover these expenses. There’s so many reasons why this is wrong, but the biggest one is if you’re following your entrepreneurial passion, it’s much easier if you don’t have children. Health costs, rent costs, mortgage costs, daycare costs, educational costs, taxes, etc…. are all anti family.

Wife and I were talking just the other day about possibly not getting insurance and self insuring. It’s a gamble, if something major happens. But otherwise, we most likely wouldn’t come close to spending $30K in a yr on healthcare.

Back in the day when I was a kid, 1st grade or so, I was staying weekend with my Grandmother who could not drive. We were baking pot pies in the oven when I got ahead of myself and dumped hot pie on my hand. Needless to say it called for a doctors attention. Grandmother called her doc who delivered my mother and me and he drove out to the house to treat my burns.
Those were the days of compassionate care …..we always paid out of pocket ….it was within reason. Being a doctor was actually a calling instead of a lucrative career. ….Just think if people had the disposable income that they shell out for healthcare and how it could affect personal lives as well as the economy.

Long ago, I had a doctor who gave a cash discount! The cash went straight into his pocket. Better have exact change!
I got 20% knocked off a dental bill just because i asked for a discount and paid cash. No dental insurance. That amounted to an over $300 discount. The worst the doctor can do is say “no”, so I always ask.
Many dentists don’t take insurance but discount for cash vs credit card the percentage of the credit card service fee they used to absorb.
The trade off is getting close. The expenses are deductible which helps. It’s the catastrophic risk I’m looking to cover. Our kids are 20, so when they drop off in 3-5years, our grandfathered plan will go away.

Dude, that is a lot. I had a community share plan for the family of 5 earlier this year, it was $650 per month and $7500 family max deductible. Luckily I got a job and it covers full premium now. But if your family is healthy you should not be paying that much.

Most of us have heard about the method for corralling hogs. First pile food in the middle of a field. The hogs flock to the free food. Every day, add more food and, while the hogs feast, add a section of fencing around the food. The hogs keep coming until the day that the final section of fence is installed. And like that, the hogs are corralled and there is no further need to add food.
And so it is with people and their government. The people are conditioned by their government over time to accept the most outrageous treatment. We are corralled, but the smart hogs may soon realize that they have the power to break down the flimsy fencing, freeing themselves.

Like most abuses, a kernel of truth (doctors do make egregious mistakes and shouldn’t just act with impunity) drove a storm of suits and in turn insurance agencies quickly developed insurance so that doctors could practice without going under the first time someone had enough resources to conduct lawfare against them.
All insurance controls cost by limitation of exposure to risk; so from that moment on medicine stopped being about making a patient better and started being about doing what the insurer demanded to keep being protected. The only shield against a shark frenzy of lawyers is the insurance, and the insurance lives or dies based on managed risk. This is all mathematical and reasonable and entirely predictable but deadly to any attempt to make people better.
Pressuring on the other side are ever increasing benefits to the elderly, to the infirm, to the poor, all with capped cost; so as the risks increase and insurance demands limitation of risk, there’s also caps on gross income — so the *only* way to stay alive is to stop trying to treat people and instead develop protocols. Protocols are a standard and legal defense, so limit risk; and the more complicated the protocol, the more procedures can be punched through billing caps to make money.
This is the situation we live in, and in more regimented countries like Canada and UK, die in as the ROI on helping you perish is increasingly better than even the slow grind of endless procedures (queue COVID-19, Remdesivir, and euthanasia).
Providing $ to patients is OK but misses the vital point. IMO one must strike at the heart of the hydra and quash the trial lawyers; and this is done by providing limited immunity to doctors so the burden is on the plaintiff to prove harm, burden of proof on plaintiff, costs borne by losing party, and legal penalties to lawyers to file bad lawsuits.
Until then the ravenous swarms of gimmes looking for a payout, and shark lawyers looking for easy settlement wins, cripple any attempt at real reform.
Your right about all the lawsuits and over valued settlements, it’s going on in every sector of everything, the lawyers have been allowed to get away with highway robbery. All comes back around to money lost out of all the peoples pockets is the end result.

In my experience, the generics half work or the binders make a body feel unwell. The name brand stuff is a thousand dollars without “proper” insurance. I didn’t start “L-I-V-I-N’ (sorry for the Dazed and Confused reference) until I started eating less poison. That means all forms of Doctor Dope.
Now that I “eat to live, not live to eat” I feel like I’m 27 at nearly 47 instead of pushing a hard 67 mentally and physically at 37 years old. Go Figure.
My experience as well. I didn’t start feeling really good until after stopped going to the doctors office, been years now since I’ve consulted a doctor for anything. I read up extensively on any issues that may arise and then some, preemptively treat my self with all natural 100 year old plus remedies, make sure I get my exercise in at least 3 to 5 days a week. I’m 60 and don’t look it, have zero health problem. No vaccines for over 20 years, never get sick.

Same here. And it was actually due to dramatically rising and unaffordable healthcare costs that I chose to look into alternatives, after years of being conventional and compliant. I chose to limit risk and be proactive. Surprise, surprise: I am 63, regularly mistaken for being 15-20 years younger, disgustingly healthy, take no pharmaceuticals, no vaccines, while all of my peers have this thing or that thing going wrong. Which I’m very sad about. I would otherwise wish we could all grow old together, enjoying these years, enjoying our grandchildren or retirement or second careers. I spend quite a bit of time helping out with friends and family who are having procedures or need care. It’s a weird feeling and, as I said, it makes me very sad. It is true that the more you invest in good food (which is, for the most part food in the same form in which it was created) and good physical activity (work), the less you will “need” health care and drugs. But there comes a time in everyone’s life when we need it.
The costs and methods of pricing in health care for drugs as well as everything else are criminal. Especially when the rest of the world is paying 1/10 for many of the same drugs and services.
Expose, sunlight, expose, point fingers at the individual people and companies making the decisions.
The more “needed” the health care product is: antibiotics, blood thinners, respiratory medications, diabetic medications, surgery, hospitalization.
The more wanted the product is – weight loss (weight loss drugs and procedures) , cosmetic surgery, feel good (hormones, narcotics, benzodiazepines) — the more the insurance company can refuse and people will pay cash.

I would be very OK with this, provided the HSAs can be rolled over, invested, tapped into for retirement one day, if one is a low user of the system and makes it to a certain age, or are inheritable. It shouldn’t be use or lose. Just as healthcare costs are so high now that people don’t have spare $ to save for retirement or to buy a house or education, how ridiculous would it be to amass a (today’s dollars) $500K HSA while not having any other assets? I would not mind having an HSA if I could use it to buy organic food or herbal medicine supplements, or acupuncture, or any kind of health-promoting activity. Currently, I am fortunate to have employer-paid health insurance (which certainly is factored in and lowers my salary, but nothing out of pocket towards it) which I do not use. It really is just there for catastrophic reasons, in my mind.

The problem is that this is not a practical proposal. How do individuals expect to fight high hospital bills? I tried it, it is hard to go against an army of lawyers.

If employers stop paying money to insurance companies, if patients stop paying money to insurance companies, if the government stops paying money to insurance companies, the insurance companies can’t deliver health care but the providers can.
Look at an ER bill of $30,000 that insurance pays $1200 on and you pay $800 and call that fair play. Why is there a $28,000 write-off of the original billed amount with no further payments made?
The problem with sending cash is that many low information people will not use it for healthcare. I think they will waste it and end up in the emergency room for treatment when needed, without being able to pay of course.

Set up a national HSA “Lock Box” with contribution maximums based on a percentage of adjusted gross income. Maybe 5%. So if you make $100 K per year, you can contribute and write off $5 K off your taxes. If you make $10 MM per year, you can contribute and write of $500 K off your taxes. Let the money accumulate as a pool the way social security used to. If you need to visit a doctor for a minor illness or injury, you pay them with your federal HSA debit card. If you have a serious illness or injury that wipes out your HSA balance, the federal HSA pool kicks in and pays the provider a percentage of whatever is left over, based on the percentage of the maximum HSA contributions that you paid into the pool for the prior year. So if you maxed out your HSA at 5% of your income, the federal pool picks up 100% of the balance. If you have income but choose to pay nothing into the federal HSA pool, you get nothing . If you have no income and pay nothing into the pool, you’ll get whatever you get from medicaid now.

After the Flexner report and decisions were made to build health care actions on good science, specialties developed. Allopathy (MD) rose to the top over osteopathy (DO), Homeopathy, naturopathy, chiropractic, and others. Insurance and government insurance led to governing bodies (FDA) who determined which treatments and drugs were scientifically sound.
Specialists arose: surgeons, internists/vs pediatrics, then supspecialists: cardiology, pulmonology, oncology, gastroenterology, urology, proctology, nephrology, dermatology, Otolaryngology (ENT), Endocrinology.
For primary care we couldn’t get the pediatricians and internists out in the rural areas.
Then in 1968 the specialty of family practice was designated: 4 years after med school of additional education but still focused on being a generalist. Care for 80-90% of problems, coordinate the rest, have continuity, know the whole patient and family, see the patient in the office, home, hospital, ER, deliver babies, care for kids, cradle to grave.
A knowledgable physician who walks with the patient through all the health care settings, who earns trust of the patient and family through the lifespan, lives in the community, knows the patient in their own community settingl — is a powerful advocate for a patient.
The most powerful advocate for us , however is an educated wise patient, who is a good steward of his/her own health, and has the power to purchase and pay for and make decisions on what they need. A primary care doc, especially a family doc, is the best advocate for a patient to make good common sense decisions about their health.
I answered the call to be a Family Doc. As I taught medical students I told them, “Family Medicine is the Navy SEALs of medicine”.
In the 90’s I went to a small town, delivered babies, saw my patients in the office, in the ER, at home, in the nursing home. I was able to do it and make all the games and coach a little for my kids. We were powerful advocates at the ground level in the trenches with our patitients. We were first to see when the “science” was bad or a lie. We had the power to direct care in the hospital, in the communty. Advocating for patients.
Empowered primary physicians are a threat to the grifting of the medical industrial complex, and they have all but neutered us. Primary Care is almost non viable. By design.

Sorry to bring this up, but it needs to be part of the health care conversation. Many Americans have unrealistic expectations concerning health care. Especially geriatric end of life care. Some refuse to accept the limitations of human mortality and expect others to participate in the delusions at great cost.
I came to this conclusion after being involved in 5 hospice end of life events in a short time frame. The levels of denial were astonishing, all kept pretending there was some pill or procedure as fountain of youth. I have made my own personal decisions with the above in mind.
Obama’s un-Affordable, no care for citizens, Care Act was always a process to funnel trillions of dollars to insurance companies so they could funnel it back to democrat politicians while forcing the country into a Socialized Health Care System. The only way health care in America can be saved is for the un-affordable care act to be totally repealed and the following be made law:
1 – individuals and employers can buy health insurance from any entity they choose
2 – nobody is required to buy health insurance
3 – no state or local government can require any coverages be included or require minimum coverage levels
4 – all health care providers must provide the cost of all care proposed to patients/customers upon request
5 – elimination of the PBM system for pharmaceuticals and allow individuals to purchase what they need directly from pharmaceutical manufacturers
6 – require Medicare and Medicaid to negotiate directly with pharma to get the lowest drug costs for consumers
How about Congress passing laws where insurance companies can’t rake in hundreds of billions in profits while denying coverage for people paying premiums ?

Health insurance is not a right to anyone and handing big bucks to politicians should be illegal and I’d bet there already is illegal aspects to that.
When Rand Paul said Tariffs are a tax on the citizenry and illegal, my first thought was the ACA that required purchase of health insurance…which feels like a tax to me. Perhaps Trump is implying the similarity between the two court cases brought to the SCOTUS.
They repealed the tax portion a ways back. I don’t think there is any way to enforce the “mandatory insurance” now.

Many medical practices have two rate scales …. a scale for patient cash payments that charges for services ONLY and another scale where they have to ADD in the overhead added by Health Insurance Companies and US Government if on Medicare (which is required if 65 or older).
Some form of HSA Account for patients to draw upon, to DIRECTLY PAY MEDICAL BILLS, would be an interesting option for than just those on Obamacare. The trick will be how to cover procedures beyond basic and non-surgical coverage.

I pay 40-50k in premiums, deductibles and non-covered expenses like vision and dental annually. Those expenses were around 12k thirteen years ago. Based on your comments above, I’m guessing you’re not paying much out of pocket annually?

What state and how many on policy? My wife retired early in MN and paid about $650 a month without dental and hearing until she got on medicare. So, about $8,000 a year for a policy is still too high, but $40-50k?
As some are saying, when the premium is higher than taking the risk of zero insurance, somethings got to give. Even a family of 4 with some minor medical issues wouldn’t spend $40k a year. Insurance is always about what we are willing to risk.
It all comes back to supply and demand. When the government subsidizes something supposedly making it cheaper, it increases the demand, allowing suppliers to charge more. It’s just a pass through money launder and really doesn’t help with a student loan or health insurance or mortgages or farmers and crops or even groceries. If everybody had SNAP, groceries would be even higher. What is the incentive of a supplier to reduce prices when the demand is driven by ever increasing supply of easy money?

Ok. Here’s the deal. Each of us has a responsibility to stop idolizing doctors and pills. If I go into the doctor’s office and complain about my hip, I’m eventually (after many expensive tests and beautiful pills) going to end up with $80,000 of hip surgery that may or may not make it better. Maybe the original doctor is trying to be kind and responsive. So it is I who have the initial responsibility to take care of myself and try to avoid medical care.
So what each of us, hundreds of thousands of us, should do today is do everything we can do to take care of ourselves and our loved ones. Be determined to not know the location of the pharmacy. We all have the ability to research alternatives.

I have worked for State Medicaid in a blue State for years. I thought I was helping my fellow most vulnerable citizen. What I have seen is American developers/coders systematically be replaced by H1B immigrants primarily from the middle east (Hindus & Muslims). They code what they are told to code and don’t cause a fuss when the rules they are coding only help immigrants and illegals. None of the functional analyst, system analyst or testers complained because they were not developers. Well Deloitte just signed a contract in my blue State to replace the remaining American Citizens. This was after they just laid off approximately 1000 citizens in State Government. Illegals and H1B visa holders are now in charge of our country.
We have H1B immigrants that hate the Citizens of our Country working in all levels of government (City, County, State, Federal, Center for Medicare & Medicaid, United States Customs & Immigration (USCIS), Courts). One developer told me one line of coding with an effective date and time could turn off all airplanes at the same time. It would just need to be hidden in an area of code that never got reviewed.
We need to replace every H1B visa holder in the country with an American born citizen. This is a matter of national security as well as national economics.
In addition this same blue state will use self-attestation for citizenship, to allow illegals to access Medicaid and they will submit federally approved emergency billing codes to make sure all illegals get full Medicaid paid for by tax payers. They have workarounds for everything.
