For historical perspective it dates back to hand set type, for newspapers.
They decided it was proper, the first time they refer to the POTUS, in a Story (headline doesn’t count) to refer to him as President blank, but then after that, any additional time to just refer to him as “blank” to save the typesetter having to spell out “President” every time, and save space as well.
As TV developed, they continued this ‘journalistic practice’ as well.
With our recent history of both partisanship, and 8 year Presidential cycles, people from one party don’t notice a President of the other party being referred to by his last name, or taking offence at it for his 8 years in office.
But, when THEIR guy gets in, they notice it and think their guy is being slighted.
I recall libbies B & M’ing at talking heads referring to Obama, by last name only, not noticing the head said “PRESIDENT Obama” the first time.
Now that we have made the move from TV, to include the internet and phones like I am typing on, there are all sorts of “shorthands” and LOL type things being used.
I compromise with “PDJT” most of the time and don’t do the whole “President” the first time, because I am not a journalist.

I come from a different background… raised in a different culture where in my native language even the way we address someone elder than myself had a different word to show respect. As far as I know, here we use the word Mr. or Mrs. to convey the same respect. But the person himself or herself will never use the word Mr. or Mrs. to address himself or herself when using their own Last name.
The onus to show respect is always on other person. Stop blaming President Trump for your lack of judgement on this. You may feel like he is your buddy and best friend. Simple question: do you call your Dad in public using just his last name or as Mr. when you are in a professional setting? i.e., not at home when you can address him as Dad.
Do not weasel your way out with the excuse that even his own sons (Eric and Don Jr.) address him as “Trump” so why should I?
