In what is being billed as the largest single takedown of HHS fraud, today the DOJ announced indictments for a massive $14.6 billion Medicaid/Medicare fraud program operated out of the U.S and several foreign countries.
DOJ PRESS RELEASE – The Justice Department today announced the results of its 2025 National Health Care Fraud Takedown, which resulted in criminal charges against 324 defendants, including 96 doctors, nurse practitioners, pharmacists, and other licensed medical professionals, in 50 federal districts and 12 State Attorneys General’s Offices across the United States, for their alleged participation in various health care fraud schemes involving over $14.6 billion in intended loss. The Takedown involved federal and state law enforcement agencies across the country and represents an unprecedented effort to combat health care fraud schemes that exploit patients and taxpayers.
Demonstrating the significant return on investment that results from health care fraud enforcement efforts, the government seized over $245 million in cash, luxury vehicles, cryptocurrency, and other assets as part of the coordinated enforcement efforts.
As part of the whole-of-government approach to combating health care fraud announced today, the Centers for Medicare and Medicaid Services (CMS) also announced that it successfully prevented over $4 billion from being paid in response to false and fraudulent claims and that it suspended or revoked the billing privileges of 205 providers in the months leading up to the Takedown. Civil charges against 20 defendants for $14.2 million in alleged fraud, as well as civil settlements with 106 defendants totaling $34.3 million, were also announced as part of the Takedown. All healthcare should be fee based, market competition, user review driven, and insurance companies competing nationally to sell catastrophic health insurance policies. Money spent on healthcare and catastrophic insurance would be 100% tax deductible to incentivize full participation to lower the cost for everyone. Aggressive gov’t policies educating the populace about eating healthy and exercising and penalizing people that don’t with higher premiums.
Individuals would have the option of upgrading to Cadillac policies if they choose. Charitable organizations would set up to treat the poor and indigent with healthcare workers able to deduct their volunteer time against their income taxes.

However, given the heavy reliance of our current medical system on expensive imaging, robotic, and other equipment, and on lab-based diagnostic equipment and the trained personnel to properly run it all…the transition from now to the way things used to be might be a long and painful process.
Many of the expensive tests you mention are not needed nor are they necessary… Your Dr. sends you for the test, knowing you will have to book a “folllow-up” visit to the Dr. to hear the results of the test… Follow-up visits = $$$ in the doc’s pocket…
I’m going to somewhat disagree here. Twice medical tests for one thing exposed an entirely different and more serious problem that the original that caused the testing to begin with. The last time was a bone cancer diagnosis that would have been unknown until I broke a bone due to cancer damage. We caught it early and it is not only under control but receding. All due a scan for potential abdominal bleeding.

In general, good ideas. However, I have to tell you that there is no such thing as a univrrsal “healthy diet.” Different people can have extremely different dietary needs. What has kept me alive and healthy fot 81 years would probably be a disaster for a great many people.
The basics are universal. Special needs would be addressed by a well trained GP.
We move away from a pharma model to a healthy lifestyle model. All diet, exercise and homeopathic remedies tried, pharma is last desperate resort. If a person is on pharma, the vast majority of the time (not all the time) that person is failing in making the right choices or their doctor is failing.
This calendar year, there will be an estimated 317,000 new cases of breast cancer diagnosed. Let us know how these newly-diagnosed people will have failed to make the right choices.
Happy to. Provide their historical food and beverage choices. Provide history of drug, legal or illegal, use. Provide their exercise history. I will find it in those records what choices led to the diagnosis.
For tiny minority of them genetically guaranteed to get breast cancer, they should be treated as best as possible by any means available.
That sounds extremely invasive and a violation of our rights. I do NOT want some pencil neck requiring knowledge of my daily habits, what I eat and etc. before I can get medical care.
I am a breast cancer survivor and let me tell you, receiving that diagnosis was DEVASTATING and the treatments invasive… having someone question my habits would have been an added burden and more invasion. Lying with your breasts exposed on a radiation treatment table with people wandering in and out is enough trauma. Having poison pumped into your blood and consequent reactions…. and losing ones hair. ENOUGH to deal with let alone some dweeb wanting to know what I eat, what I drink… whether I run or walk or sit too much. Not to mention having to get an authorization for 35 daily radiation treatments, authorization for every chemo treatment…. good grief it is exhausting to have cancer!

As an aside, years ago Bloomberg in NY wanted a soda tax and (disingenuously?) said it was necessary to offset the load that diabetes would play on future NYC healthcare costs. Have to admit, I generally agreed with the concept. If they wouldn’t ‘ break the chain ‘ then they needed to control costs.

For example, now doctors are basically drug pushers. The health market is pretty much laser focused on drugs and surgery If they don’t have a drug to “fix” you and surgery won’t do the trick, “health insurance” is almost meaningless. You’re on your on and for people who are already sick, and who have a complicated illness, this can be challenging.
What doctors don’t do is provide knowledge and support for people who could benefit from behavioral changes and healing modalities other than pharmaceutical drugs and surgery. If people don’t educate themselves about diet and exercise, for example, they miss out on what could be “free” cures.
Many “treatments” other than drugs could help many people. An example is acupuncture/accupressure, very little of which is covered on most policies and it is difficult for people to educate themselves. Another example is nutritional counseling, which supports people with specific health issues and seniors, but Medicare doesn’t pay for it unless someone already has a serious illness (which means drugs are once again involved). Another example is physical therapy for people with complex issues, but now those kinds of appointments are limited in most policies.
The bottom line is we need to rethink how to provide medical care and one of the key aspects is to provide knowledge and support for non-drug related interventions. Doctors should be aware of all treatment possibilities and impart that knowledge to patients.
The medical establishment often claims that drugs are needed because most people won’t do the work to change behavior and they want quick fixes. But the real problem is people aren’t aware. They are not given the information they need to make a true choice. Although covid has woken up a lot of people and hopefully Secretary Kennedy will do even more.
Not sure I’m on board with the aggressive gov’t policies and penalties part, but most everything else is the way it was when I growing up. Everyone should still have the freedom to make choices for themselves, along with the chance to experience the consequences of those choices, whether they be good or bad.
And health insurers might take note of the common-sense policies of some homeowner insurance carriers… if you insist on building on an earthquake fault that runs through the middle of a wildfire-prone flood zone, you’d better be self-insured.

In their medical training they are taught that we are not a whole with mind, body and soul, but parts. Now a days, you don’t go to a doctor, you go to a specialist who only treats one part of the machine. As for those charitable organizations, kindly name a few, or even one. What charitable organization has a retinal surgeon who is board certified to treat macular edema? Or has a board certified urologist to treat prostate cancer? Or has an integrative radiologist to treat ileac compression?
None of these conditions is related to poor lifestyle choices and each requires a specialist, not some volunteer looking to get a break on income taxes. Medicare and medicaid are not Healthcare they are insurers. Tied to Social Security and Welfare. Which I’ve paid into for 50 years so not sure how you just take it away. Welfare is another matter all together. But taking help from the truly impoverished never really goes over to well.

There is a LOT of unConstitutional stuff going on from our Government. I don’t see how DOEd, for instance, isn’t a direct violation of Amendment 10. Frankly, just about any 3-Letter or department/program that isn’t specified as a Federal function in the Constitution should be considered unConstitutional. And, for some reason, all these types of programs are super expensive and more out of control that the functions of the Federal govt. that are identified in the Constitution.
My wife is Thai. Healthcare is better in Thailand. I wasted two hours today; trapped between my insurance company and a Dr. It sucks, and it is the governments fault.

We’ve reached the absurd point where spending on healthcare continues to rise even after average life expectancy has fallen.
Many of the doctors are foreign medical grads, that get MBBS instead of MD and rob a US citizen of a limited spot in a Residency program here. Many become Dr.s just to exploit our systems to become wealthy. They are entrepreneurs not humanitarians hoping to help heir fellow man anymore. Many from India, Pakistan, Africa.
Health care in the US is a scam. It is not focused on patient care, it’s focused on billing and what medicare is willing to pay. Focussing on billing is what allows the fraud.
If you look, it’s possible to find healthcare providers that are “wellness” based. Medicare will pay for SOME of the costs. We are lucky to have one near us. It’s focused on causative issues, not just treating symptoms (with drugs)
Trouble is they are opting out of Medicare and insurance plans…payment at each visit and then you file your own claims. So now I’m paying for health coverage out of my monthly Medicare yet have to deal with trying to get some of my expenses back…not a happy camper on this new trend.

He has been taken by ambulance to the hospital two blocks from where he “ holes up” at least eight times.
He drinks and takes pills then when the pain gets too much has a passerby call him an ambulance.

Yet an individual can sit in the emergency ward at the same hospital for eight hours or more.

I’m in my late 70s with some health conditions and to this day I’ve only had one Doctor recommend anything but prescription pharmaceuticals! I have Wet Age-related Macular Degeneration in one eye and dry in the other. My eye doctor and my retina specialist both recommended I take Areds2. All the other supplements I take were mostly recommended by my older brother, but I didn’t take his word for it I researched them myself!! My heart health is better for taking them. What GB has posted is known but is anyone in Congress or the medical community working on changing that system? I doubt Congress would or they might lose their Big Pharma donations!!! Here is a problem with a known solution but nothing get done such is the power of Big Pharma!!! Money talks bull💩walks!!!

If D.O.G.E. fulfills it’s mandate to root this kind of waste and fraud out, and the agencies and programs and services become efficient AND effective, our country might actually end up eliminating the debt!

Zero tolerance needs to become the only standard. Stealing from the government needs to be prosecuted to the harshest standards.
We need to see the defendants lists, where they are and who the employers are.
Are these merely individuals gaming the system, or are some coordinating with each other, and are there deeper ties to the hospitals, clinics, practices, agencies to which they are connected or employed.

All Medicaid participants should be required to use HMOs and the government simply pays the monthly premium. No over billing and there is no incentive for participants to fraudulently over utilize medical services since the government isn’t paying for doctor’s visits.
Maybe the Health Care Fraud Unit’s Data Analytics Team can be placed on – loan to the DOJ attorney in charge of investigating election/voter fraud crimes.

Read all 81 posts and was surprised to see (not!) none giving kudos to the constantly maligned DOJ for “not doing a damn thing” except go on talk shows. And, of course, the recent Supreme Court just happened by magic, too.

It’s about time somebody got serious about this fraud. There have been many investigateive articles about this kind of fraud, written by conservatives of course, ignored by the media and denied by the government for many decades.
Since DOGE was not allowed access to the payment systems in the Treasury Dept, they were not able to crosscheck to see how many of those hundreds of thousands of Social Security members over 120 years old listed as “Active” to determine if they were not actually still being paid.
Perhaps while looking you might consider such agencies as those who are responsible for disability. Many people are given disability because the agency is required to meet a goal. That don’t seem right. While disabilty is a state run the payments come for SSA.

I remember sitting in the back hallway listening to a clinical trials investigator talk about his case(s), just one teeny little niche in the miasma of medical fraud, and the work that went into that. This is more than I can even being to wrap my brain around. Just freakin’ Awesome.

Medicaid should only provide benefits via HMOs!!! The government is only responsible for paying the monthly HMO premiums based on competitive bidding and verified eligibility and is not responsible for paying doctors, hospitals, clinics or pharmacies. There’s no way doctors or hospitals can bill the government for fraudulent medical services and fraud is totally eliminated! We save trillions of dollars and simply verify that the HMOs provide necessary medical care! 🫡. Of course, it would also work for Medicare but that would be too political and run into far more opposition.
$4,295,00,000 is less than a 3rd. It’s far less if the $4B is not considered in the $14B+ headline number.
