Hard to know the real story. Could be sinister or simply unethical people scared of losing their jobs. I’ve seen chemical industry contractors place great pressure on subcontractor x-ray companies to approve all welds (despite the high “death in pipes” stakes). Often companies simply fire the guilty parties and seek to avoid further publicity. Criminal charges, where warranted, would be a nice change.
